Wellness is defined as an optimum state of health: physical, emotional, psychological, social, spiritual and financial. It is influenced by family health history and past personal health history. Wellness can be increased by activities in health promotion, illness and injury prevention, self-care education and treatment.

Health promotion involves improving and maintaining healthy attitudes, nutrition, active living, personal environment, and spirituality.

Illness and injury prevention consists of personal illness and injury prevention as well as periodic health examinations with appropriate screening.

Self-care education teaches individuals to take care of minor illnesses and injuries and to know when to seek professional help. It also assists in personal management of any chronic disease.

Treatment consists of professional care from a variety of providers when required.  Improved wellness can give you a longer and healthier life, increased self-esteem, more energy, and decreased illness and injury. A wellness evaluation can indicate areas where improvement can be made. 

This site is meant to provide a wellness evaluation and report. It also provides advice on how to increase your wellness. You can use this wellness evaluation, report and advice to increase your own personal wellness. You can also use the report to enhance your resume or for pre-employment screening. 

Complete the questionnaire, it takes about 20 minutes. You can get your blood pressure and other blood work (sugar and cholesterol) at a pharmacy, walk in clinic or family doctor.
